Children between 2 months to 18 years are eligible if they: are American Indian/Native American or Alaskan Native, do not have health insurance, have Medicaid or are under-insured (insurance does not cover vaccines). Under 18 years children must be accompanied by a parent or guardian. CHIP is not accepted. Please bring an original shot record.
