Methodist Hospital Metropolitan Expands Downtown San Antonio Campus With New Operating Rooms, 969-Space Parking Garage and Community Murals
For San Antonio families who depend on downtown medical care, a hospital expansion can mean more than a bigger building. It can mean improved access to surgical services, a more convenient place to park during hospital visits and a better experience for patients and loved ones navigating an already stressful day.
Those improvements are beginning to take shape at Methodist Hospital | Metropolitan, where three new operating rooms, a 969-space parking garage and three large-scale community murals have marked the latest milestones in a $200 million campus expansion.
The project, announced in 2025 and expected to be completed in early 2028, represents a significant investment in healthcare infrastructure in downtown San Antonio. It also brings something less expected to the hospital campus: original artwork created by local artists with help from the community.
New Operating Rooms Expand Surgical Capabilities at Methodist Hospital Metropolitan
The hospital celebrated its latest expansion milestones on October 22, 2026, bringing together physicians, hospital leadership, local artists and community partners at its McCullough campus.
Among the most important additions are three new operating rooms, including a hybrid operating room designed to support advanced surgical procedures and cardiovascular care.
Hybrid operating rooms combine traditional surgical capabilities with advanced imaging technology, allowing medical teams to perform certain complex procedures in a setting equipped for both surgery and image-guided treatment.
For patients in San Antonio and surrounding communities, the additional surgical facilities represent an investment in the hospital's ability to provide specialized care as local healthcare needs continue to evolve.
"Every part of this expansion is about creating a better experience for the people who count on us," said Greg Seiler, chief executive officer of Methodist Hospital | Metropolitan. "From expanding our surgical capabilities to improving access and creating a more welcoming environment, we're investing in the future of healthcare for our patients, their families and the community we are proud to serve."
New 969-Space Parking Garage Improves Access for Patients and Visitors
Anyone who has accompanied a family member to the hospital knows that finding parking can be one more concern during an already difficult day.
The newly completed 969-space parking garage at Methodist Hospital Metropolitan is intended to improve access for patients, visitors and others traveling to the downtown San Antonio medical campus.
The garage opened for patient and visitor parking on Friday, October 23, following a ceremonial celebration the previous day.
During the event, longtime Methodist Hospital Metropolitan physicians Dr. Daniel Juarez and Dr. Randall Rogers led the first ceremonial drive into the garage, marking the completion of one of the expansion's most visible improvements.
San Antonio City Councilwoman Sukh Kaur, who represents District 1, and Trish DeBerry, president and CEO of Centro San Antonio, joined hospital leaders and community partners for the milestone.
While the parking garage is a practical addition to the campus, its exterior has also become a place for San Antonio artists and residents to leave their mark.
San Antonio Artists Bring Community Murals to Methodist Hospital Metropolitan
Three large-scale murals now bring color and locally inspired artwork to the hospital campus, connecting the expansion to the surrounding downtown San Antonio community.
The murals were designed by San Antonio artists Malachy McKinney and Sandra Gonzalez, whose work was selected through Centro San Antonio's Art Everywhere initiative.
The project went beyond commissioning artwork. Community members, YMCA participants and hospital colleagues took part in painting events leading up to the installation, helping turn the murals into a shared community effort.
For those who participated, the finished pieces offer a lasting connection to a place where countless San Antonio families have experienced important moments in their lives.
The murals also reflect a broader effort to make the hospital campus feel more welcoming, particularly for patients and visitors who may be arriving for difficult medical appointments or supporting loved ones through treatment.
Hospital leadership recognized McKinney and Gonzalez during the October 22 celebration, acknowledging both the artists' contributions and the community participation that helped bring their designs to life.
Centro San Antonio's Art Everywhere initiative works to incorporate public art into the city's urban environment, creating opportunities for local artists while adding character to shared spaces.
At Methodist Hospital Metropolitan, that approach has brought public art into a setting where people come not only for medical treatment, but also for reassurance, recovery and time with family.
What Is Included in Methodist Hospital Metropolitan's $200 Million Expansion?
The recently completed operating rooms and parking garage are part of a larger hospital improvement project that will continue over the next several years.
Announced in 2025, the $200 million expansion includes a vertical expansion of the hospital's Women's Pavilion, additional patient beds and expanded surgical services.
The Women's Pavilion expansion is particularly relevant to San Antonio families seeking women's healthcare services, while the additional patient beds and surgical facilities are intended to strengthen the hospital's overall capacity.
The project includes several major components:
-
Three new operating rooms: Including a hybrid operating room supporting advanced surgical and cardiovascular procedures.
-
969-space parking garage: Opened October 23, 2026, to provide additional parking for patients and visitors.
-
Women's Pavilion expansion: A vertical addition to the existing hospital facility.
-
Additional patient beds: Planned as part of the hospital's expanded healthcare capacity.
-
Community murals: Three large-scale public artworks created by San Antonio artists with participation from local residents and organizations.
-
Expected completion: Early 2028 for the overall expansion project.
The hospital has not provided a detailed opening schedule for every remaining component in the information released about the October milestone.
More Than 50 Years of Healthcare in Downtown San Antonio
For more than five decades, Methodist Hospital | Metropolitan has served patients and families in downtown San Antonio and neighboring communities.
Over those years, the hospital has become part of the city's healthcare landscape, caring for generations of residents through routine procedures, major surgeries and significant family milestones.
Its location also places it within an established urban neighborhood where hospitals, businesses, residents and community organizations share space.
That connection helps explain why the latest expansion includes both clinical improvements and public art.
The project is not simply about adding medical facilities. It also reflects an effort to improve how patients and visitors experience the campus, from arriving and finding parking to entering a hospital environment that feels connected to the city around it.
For downtown San Antonio, continued investment in an established medical campus also contributes to the ongoing development of the surrounding area, although the hospital has not announced specific economic or neighborhood development projections associated with the expansion.
What's Next for Methodist Hospital Metropolitan in San Antonio?
With the new operating rooms and parking garage completed, attention now turns to the remaining phases of the hospital's $200 million expansion.
Work will continue on the Women's Pavilion, additional patient beds and other planned improvements, with the overall project expected to be finished in early 2028.
For patients and families, the most immediate changes are the additional surgical facilities and newly available parking. Over time, the remaining construction is expected to further expand the hospital's capacity to serve the community.
And for the San Antonio residents who helped paint the murals, the completed artwork offers something that will remain long after the construction crews have finished: a small piece of their own community's creativity woven into a place dedicated to caring for others.
